Many patients come to Evergreen through physician and hospital referrals. Upon admission, an interdisciplinary team of health professionals develops a plan to help residents recover as quickly as possible and regain the skills they need to live on their own again.
The team includes physical therapists, occupational therapists, registered nurses, licensed practical nurses, certified nurses and a full-time nurse practitioner, who can diagnose and treat chronic illnesses and prescribe medication. Evergreen also contracts with a variety of other medical and health professionals, including licensed counselors, a geriatric psychiatrist, speech therapists, social workers, a consulting wound care specialist, a pharmacist and a registered dietician.
A physician, who serves as Evergreen’s medical director, is also on site three times a week to treat patients.
